Enrollment Agreement and Release for Participation in Chris Brooks’ Elite Hockey Camp In consideration to my enrollment in this hockey camp, I release and discharge all Chris Brooks’ Elite Hockey Camp agents, employees, officers, owners, volunteers and other participants on behalf of myself, my children, my parents, my heirs, and assigns as follows:

1. I acknowledge that the sport of hockey involves known and unknown risks which could result in physical or emotional injury, paralysis, death or damage to participants, myself, to property, or to 3rd parties, and that such risks simply cannot be eliminated. To that end, further acknowledge that Chris Brooks’ Elite Hockey Camp is not responsible for a participant’s fitness, abilities, or the equipment being used.

2. I acknowledge and agree to accept and assume any and all the risk attendant to this activity. My child’s participation in this activity is purely voluntary and I elect to participate notwithstanding the risks.

3. I hereby volunteer release, forever discharge, and agree to indemnify, and hold harmless, Chris Brooks’ elite Hockey Camp from any and all claims, demands, or causes of action which are in any way connected to my child’s participation in this activity or my use of the facilities, whether on or off the ice, including all claims which allege negligent acts or omissions on the part of Chris Brook’s Elite Hockey Camps.

4. In the event Chris Brooks’ Elite Hockey Camp, or anyone acting on their behalf, is required to incur attorney fees and cost to enforce this agreement, I agree to indemnify and hold harmless for all such fees and costs.

5. I certify that my child has adequate insurance coverage for any injury or damage I may cause or suffer while participating, and I agree to bear any and all cost of such an injury or damage.

6. I further certify that my child has no medical or physical conditions which could interfere with my safety in this activity, and I am willing to assume all risks and costs that may result, directly or indirectly from such a condition.

7. By signing this document, I acknowledge that if anyone is hurt or property damaged during my child’s participation in this activity, I have waived my rights to maintain a lawsuit against Chris Brook’s Elite Hockey Camp. I have had sufficient opportunity to read this entire document, and I understand and agree to be bound by it’s terms. I, have been provided and read the concussion and head injury information sheet.

8. I understand that there is a risk of injury during athletic participation and I agree to disclose any signs and symptoms of a concussion to the camp coaching staff.

9. I also understand that I will be removed from play to eliminate the risk of further injury and will not be able to resume participation until evaluated and cleared by a health care provider who has experience with evaluating and managing pediatric concussions and head injuries.

10. I will provide written clearance on the health care provider’s letterhead or prescription note allowing me to continue participation in the activity. 

11. I hereby consent to Chris Brooks Elite Hockey Camp, Chris Brooks, its coaches/ instructors and all Chris Brooks Elite Hockey Camp employees to take video footage and photographs of me during my participation in Chris Brooks Elite Hockey Camp, events, and activities. I understand that these videos and photographs may be used for advertisement and promotional purposes by Chris Brooks Elite Hockey Camp. I understand and agree that Chris Brooks Elite Hockey Camp may use these videos and photographs in various media formats, including but not limited to print materials, website content, social media platforms, and promotional materials.

I waive any right to inspect or approve the finished product(s), including written or electronic copy, wherein my likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of the video footage and photographs.

I release, discharge, and hold harmless Chris Brooks Elite Hockey Camps, its coaches/instructors, Chris Brooks, and all Chris Brooks Elite Hockey Camp employees from any and all claims, demands, or causes of action that I may have against them arising out of or in connection with the use of the video footage and photographs, including but not limited to any claims for defamation, invasion of privacy, or infringement of moral rights. I affirm that I am at least 18 years of age and that I am competent to enter into this agreement. If I am under the age of 18, I affirm that my parent or legal guardian has provided consent on my behalf.

I HAVE READ THIS RELEASE AND WAIVER OF LIABILITY, PHOTO & VIDEO RELEASE AGREEMENT & POLICY TO PLAY AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND HAVE SIGNED IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT, ASSURANCE, OR GUARANTEE BEING MADE TO ME AND INTEND MY SIGNATURE TO BE COMPLETE AND UNCONDITIONAL RELEASE OF ALL LIABILITY TO THE GREATEST EXTENT ALLOWED BY LAW. 

By Registering, you have read the above carefully and agree to everything stated. 

I have read this Photo & Video Release / Consent Form & Policy To Play Form and understand that I have given up substantial rights by agreeing to it. I agree to this waiver voluntarily, intending to be legally bound.
